IN-HOME SWIM LESSON WAIVER & ASSUMPTION OF RISK

I understand that swimming and aquatic activities involve inherent risks, including but not limited to slips, falls, collisions, muscle strains, water inhalation, drowning, serious injury, illness, and, in rare circumstances, death.

By participating in swimming lessons, I acknowledge and voluntarily accept the risks associated with aquatic activities.

I confirm that I have disclosed any medical conditions, allergies, injuries, disabilities, medications, or other information that may affect the participant’s ability to safely take part in swimming lessons. I understand that it is my responsibility to update the instructor if this information changes.

For lessons taking place at a private residence, I confirm that I am authorized to permit use of the swimming pool and surrounding property for the scheduled lesson. I am responsible for ensuring that the pool, pool deck, access areas, water conditions, and related equipment are reasonably safe and suitable for swimming.

I understand that the instructor may delay, modify, or cancel a lesson if, in the instructor’s judgment, the pool, weather, water conditions, participant behaviour, equipment, or surrounding environment creates an unsafe condition.

For participants under 18 years of age, a parent or legal guardian must provide consent for participation. I understand that swimming instruction reduces risk but cannot eliminate all risks associated with being in or around water.

In the event of an emergency, I authorize the instructor and/or company staff to provide reasonable first aid, CPR, use an AED, contact emergency services, and take other reasonable emergency measures when necessary. I understand that any costs associated with ambulance transportation or medical treatment remain the responsibility of the participant or parent/guardian.

To the fullest extent permitted by applicable law, I voluntarily assume the inherent risks associated with participation in the swimming lesson and release FASTLANE SWIMMERS, its instructors, employees, contractors, representatives, and agents from claims arising from the ordinary inherent risks of participation, except where liability cannot legally be excluded or limited.

I agree to follow all safety instructions provided by the instructor. I understand that failure to follow safety directions may result in the lesson being stopped without completion.

By signing below, I confirm that I have read and understood this agreement, have had the opportunity to ask questions, and voluntarily agree to its terms.
